Wound Care Nurse - Home Healthcare
Location: NYC
Job Type: Full-time, Monday-Friday, 40 hours per week
Job Description:
We are seeking a dedicated and compassionate Wound Care Nurse to join our dynamic healthcare team. As a Wound Care Nurse, you will play a crucial role in providing specialized care to patients in the comfort of their homes or skilled nursing facilities (SNF). This position requires a commitment to excellence in wound care, strong communication skills, and the ability to collaborate with a multidisciplinary team.
Key Responsibilities:

Patient Visits:
- Travel to patients' homes and/or SNF facilities within the state where licensed.
- Coordinate and schedule evaluations and visits in collaboration with nurses and providers.
- Perform wound treatments and dressing changes with precision and care.

Communication:
- Maintain continuous communication with patients, families, and caretakers to ensure comprehensive care.
- Confirm appointments and approximate time of visits to enhance patient and family experience.
- Act as a liaison between caregivers/patients and healthcare providers.

Durable Medical Equipment (DME) Management:
- Ensure all DMEs required for patient care are ordered promptly.
- Oversee the delivery of DMEs to patients, ensuring they have the necessary equipment for their care.

Documentation and Reporting:
- Complete thorough and timely documentation in accordance with established policies.
- Review wound progress on a weekly basis and generate comprehensive weekly reports.
- Assist with chart/note reviews and audits as necessary.

Educational Role:
- Provide expertise in wound care, serving as the go-to person for wound-related inquiries.
- Assist in creating, reviewing, and updating policies and procedures related to wound care.

Collaboration:
- Collaborate with the healthcare team to ensure coordinated and holistic patient care.
- Participate in weekly discussions with families, caretakers, or patients to address concerns and provide updates.

Flexibility and Team Support:
- Assist the company with any additional tasks or responsibilities as needed.
- Contribute to a positive and collaborative work environment.
Qualifications:
- The current nursing license is in NY.
- Wound care certification or relevant experience preferred.
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Ability to adapt to changing situations and priorities.
