Job Description:
We are looking for our client an esteemed endocrinology group. They specialize in providing exceptional care to adult patients with a focus on type 1 and type 2 diabetes, as well as weight management. The chosen candidate will have the unique opportunity to travel to skilled nursing facilities located in Staten Island and Northern New Jersey, delivering specialized endocrinology services.
Responsibilities:
Diabetes Care:
- Provide expert care to adult patients with type 1 and type 2 diabetes.
- Conduct thorough assessments, formulate diagnoses, and create individualized care plans.
Medical Management:
- Prescribe and manage medications, with a specific focus on insulin therapy and other related diabetes medications.
- Monitor and adjust treatment plans based on patient response and changing health conditions.
Diabetes Technology:
- Proficiently manage and educate patients on the use of insulin pumps, continuous glucose sensors, and other diabetes-related technologies.
- Stay current on advancements in diabetes management technology and incorporate them into patient care as appropriate.
Weight Management:
- Develop and implement weight management plans tailored to individual patient needs.
- Provide ongoing education on nutrition, physical activity, and lifestyle modifications to promote healthy weight.
Patient Education:
- Offer clear and concise patient education on diabetes management, emphasizing self-care and prevention.
- Ensure patients and their families understand treatment plans and actively participate in their healthcare.
Collaboration and Communication:
- Collaborate effectively with other healthcare professionals, including physicians, nurses, and support staff.
- Maintain open communication with patients' primary care providers and other involved specialists.
Documentation:
- Maintain accurate and up-to-date electronic health records, documenting patient assessments, interventions, and outcomes.
Travel:
- Demonstrate flexibility and adaptability to travel to skilled nursing facilities in Staten Island and Northern New Jersey as required.
Qualifications:
- Experience in providing care to adult patients with diabetes and weight management concerns.

Salary : $130,000 - $140,000
