Meeker-McLeod-Sibley Community Health Services - PUBLIC HEALTH DEPARTMENT


Summary: Our mission is to promote health, prevent disease, and protect those who live, work, and play in our community. The Community Health Planner will work to engage internal and external public health stakeholders in the development, maintenance, execution, and evaluation of various department plans that address the health needs of our community and enhance operation processes of the department.



Job Responsibilities

  • Maintain accreditation status with the Public Health Accreditation Board.
  • Lead and facilitate meetings with agency staff, to analyze process and program performance, identify challenges, and use data to drive decision-making, prioritization, consensus action planning, and evaluation.
  • Maintain a performance management and quality improvement system to monitor achievement of organizational objectives.
  • Collect, assess, analyze, interpret, and use data from a variety of sources.
  • Develop public health project and program messaging appropriate for targeted audiences using social media and other public facing presentations.
  • Represent Meeker-McLeod-Sibley Community Health Services on task forces, committees, and work groups as directed by CHS Administrator. This may include participation in strategic and operational planning and promoting public health principles to meet community needs.
  • Assist in policy development, implementation, and management for continuous improvement in accordance with Minnesota Statute 145A.
  • Participate in organizational capacity building and training activities to advance public health competencies.
  • Assist with the organizations opportunities that refine, enhance the diversity, equity, and inclusion of programs and services.
  • Work collaborative across the department and the community to build support for and foster a shared understanding of importance of equity to achieve community health and well-being.
  • Perform similar duties as assigned.

Education and/or Experience

Education and/or Experience:

  • Bachelor’s Degree in community health, public health, health administration, health education, nursing or related degree.
  • Three years prior experience working in public health or a closely related field is desired.
  • Valid driver’s license.

  • Qualifications:

    • Experience and knowledge regarding program development and community change theory and methods, setting goals and objectives, establishing, and evaluating outcomes.
    • Ability to develop and maintain effective working relationships with a wide variety of individuals including CHS staff, county staff; local, state, and regional partners; community partners and stakeholders and a variety of individuals of varying ages.
    • Ability to perform according to accepted business standards in accordance with applicable federal, state, and local laws and policies and assure compliance with public health accreditation board standards.
    • Knowledge of performance management and quality improvement.
    • Ability to effectively use social media to communicate directly with community members.
    • Ability to appropriately tailor communications and communications mechanisms for various audiences.
    • Knowledge of computer word processing and spreadsheet programs including but not limited to Excel, Word, PowerPoint.
    • The ability to communicate clearly and effectively with a wide variety of individuals and groups.
    • Excellent writing skills, with considerable knowledge of English, spelling, punctuation, and vocabulary.
    • Ability to prepare accurate plans and reports.
    • Individual must have a high level of motivation, initiative, and enthusiasm with a strong and independent work ethic. Strong time management skills with organizational and prioritization skills is required.

    Language Skills: Proficient English, spelling, grammar and vocabulary skills, basic medical terminology.


    Mathematical Skills: Knowledge of basic arithmetic.

    • Work occurs in multiple environments including an office setting, meeting setting, community events, health fares, schools, etc.
    • This position is eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in an office.
    • May require work outside as the program and weather dictates.
    • Requires regular travel within Meeker, McLeod, and Sibley counties, some statewide travel.
    • Expected work hours are Monday – Friday 8:00am to 4:30pm, with some flexibility.
