Clinical Coordinator- Youth Social Worker

Salary: $51,000 - $58,000

Sign-on/Stay-on: $3,000

Do you have experience in working with individuals with behavioral or mental health diagnosis? Bring your clinical skills to a team-based workplace that puts people first. As the Clinical Coordinator, you will advance the company’s mission of providing quality service by facilitating and providing case management and support services for youth transitioning out of foster homes and into independent living.

• Provide case management and counseling services to an assigned individual caseload. Assess referred individuals and lead development, coordination, and evaluation of service plan in conjunction with treatment team, funding source, and key stakeholders.
• Maintain current and complete records in accordance with corporate, contractual, and regulatory standards.
• Provide ongoing monitoring and consultation to youth and their therapeutic teams, regarding implementation of service plan, including pre-service introduction and continuing education.
• Assist with emergency coverage and crisis intervention support as needed, including participation in rotational on-call system.
• Assist with legal, licensing, regulatory and certification activities as needed.

Qualifications:

• Master’s degree in human services; Bachelor’s or Master’s in Social Work: or Bachelors or Master’s in counseling or Family Sciences. Required.
• Maryland -Issued LBSW or LMSW Preferred.
• One-year of related experience working with individuals with behavioral and/or or mental health issues.
• Self-motivated and detail-oriented with ability to multi-task.
• Demonstrated skills with motivating team and driving results to meet goals.
• Demonstrated skills with Microsoft Office.
• Valid driver’s license in good standing with access to a registered vehicle with proof of insurance if providing transportation to individuals receiving services.
