Job Title: Documentation Specialist                                      Department:  245D and Employment Services

Reports to: 245D and Employment Services Supervisor                                Effective Date: 

 

Job Summary:

Completing and maintaining all person-served documentation for 245D for the division. 

Supervisory Responsibilities:

  • None.

Duties/Responsibilities:

  • Complete all 245D documentation required following 245D Rule timelines
  • Communicate with case managers and team members for meetings as required
  • Submit all paperwork to the proper team members and file
  • Request CSSP’s where needed
  • Perform outcome analysis for each person served according to the timeline in the CSSP Addendum
  • Frequent and routine communication with leadership on status update
  • All other duties as assigned 

Qualifications:

High school diploma or equivalent.

Proficiency in 245D documentation and timelines

Excellent communication skills

Proficient with Microsoft Suites
