Our team services a client case load with mental health diagnosis across northwestern, PA counties. We are seeking a Care Coordinator to join our team.
Position Details
The Care Coordinator is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with an Intellectual Developmental Disability and mental health diagnosis. The Care Coordinator provides intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. The Care Coordinator, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
