Position Details
Our Supports Coordinatorwill be responsible for a caseload in Lackawanna County, Pennsylvania.
- The main function of this position is to assist and support the individuals in maximizing their "everyday life."
- The principles and practices of Positive Approaches and Self-Determination will be the driving influences in the provision of support services.
- The person in this position will have the responsibility of assisting each individuals in the development of their Annual Support Plan, based on the results of specific assessments designed to develop outcomes appropriate to the individual's "personal vision."
- The person in this position will maintain regular communication and contact with each individual in order to ensure that all aspects of needed/necessary supports are in tact.Benefits

Requirements:Our ideal candidate will possess the following skills:
- The minimum requirement for this position is an appropriate Bachelor's Degree from a college of university, which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, special education or other related social sciences.
- Must also have a valid Driver's License, State Police, FBI and Child Abuse Clearances.
- Flexibility of time and work hours is necessary in this position. There may be a need for evening and/or weekend hours, dependent on the needs of the individuals receiving support.
