Hiring a Behavioral Health MSW, LSW to provide services on the inpatient Behavioral Health Unit.
Essential Job Functions: Group facilitation, Discharge planning, Community resource knowledge and collaboration, Psychosocial assessments, knowledge of involuntary processes, knowledge of guardianships, advanced directives, and POAs, Family meetings, treatment team participation, SUD screenings, SUD discharge planning, PASSR completion, Post-discharge calls, PHQ, C-SSRS, and Social Determinants of Health screenings, Crisis assessment and referral services, OPTIONAL FUNCTIONS - Care management, UR, Intern supervision, 15 minute rounding, Supervision of Social Work Staff, Individual & Group Psychotherapy in Ambulatory/Outpatient settings.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures
*1-3 years experience working at an inpatient Behavioral health facility preferred.
