Qualifications:
- Education: Associate degree in nursing.
- Licensure: Current license in applicable State of practice for Registered Nurse.
- Experience: Two years recent applicable nursing experience (senior adults and geriatric) with outstanding working knowledge of Medicare, Medicaid and other geriatric resources (e.g., nursing homes, hospice, etc.). Excellent computer skills; electronic health record skills preferred.
- Certifications: Current Basic Life Support, Case Management (or ability to become certified within 36 months of accepting employment)
- Other: Excellent problem solving, interpersonal and team player skills. Ability to establish and maintain outstanding working relationships with nursing homes and other referral sources. Projects and promotes good personal and public relations. Ability to balance multiple tasks at once and prioritize effectively and efficiently. Ability to travel by car; valid drivers license.
