Hospice Bereavement Social Worker

Overview:

The Bereavement Coordinator participates in inter-disciplinary hospice team. They are responsible for planning, implementing and maintaining a bereavement program to meet the needs of the families/caregivers of Hospice patients for up to thirteen months following the death of the patient. 

Grief support program includes: 

• Bereavement phone calls
• One on one bereavement visits
• Facilitate grief support group

Qualifications:

• Education: Master's in Social Work.
• Experience: 0-2 years of clinical experience.
• Other: Excellent interpersonal and communication skills. Knowledgeable in area of social services agencies, state regulations, and professional board standards as related to patient treatment, patient rights and client/patient confidentiality.
• Preferred Licensure: Active LMSW or LCSW in the state of employment.
• Preferred Experience: Hospice or bereavement care. 

Provides social work services for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
