The Stroke Coordinator RN is an essential component of stroke services. Stroke Coordinators provide individualized assistance to patients upon entering and being discharged from the health care system who have experienced an ischemic or hemorrhagic stroke, TIA, aneurysm repair, or Carotid surgery.
Under the direction of the Program manager and in collaboration with the multidisciplinary team, Stroke Coordinators serve as a clinical resource in providing direct and indirect care for patients aimed at optimizing patient assessment, continuity of care, education to patients and family members and facilitate ongoing treatment.
This position works closely with the Neurology, Neurosurgery, Neuro-interventional, Neuro ICU, Emergency room, and Hospitalist physician groups and collaborates with the stroke units' nursing staff, techs, therapists, pharmacists, case managers, and pastoral care.
The job duties includes :
- Coordination of acute stroke care and initiation of appropriate stroke protocols
- Participation in care team patient rounding and staff huddles
- Ensures appropriate compliance and accurate documentation of all stroke measures
- Partnerships with nurses to provide in depth stroke individualized education to patient and / or family members
- Provides discharge follow up phone calls to stroke patients that are discharged to home
- Participation in stroke team case review and performance improvement meetings and meeting preparations
- Extensive opportunities for continued education via physician rounding or formal conferences / seminar attendance
Qualifications :
Education : Graduate of an accredited School of Nursing, Bachelor of Nursing preferred
Licensure : Current License in the state of employment
Experience : 1 year acute care hospital setting, neurological care preferred
Certifications : Maintain BLS (CPR), NIHSS Stroke Scale Certification and at least 10 hours of stroke specific continuing education annually
Certifications (Preferred) :
Professional stroke certifications preferred
