Job Summary
We are seeking a Part time or Full time Family Nurse Practitioner to join our team! As a practitioner you will be examining patients, collecting patient medical history, and making tentative diagnoses on minor and major issues. You will also be reading diagnostic tests and reports, interpreting data, taking the time with each patient to explain their test results and medical diagnoses, and prescribing therapy or medication with a physician's approval. The ideal candidate has an excellent bedside manner, experience charting in a medical environment, and has strong communication and interpersonal skills. The candidate must be able to work in a fast past work environment.
Responsibilities
- Examine each patient, take a detailed medical history, diagnose, prescribe medications, and chart everything medically important in their records.
- Read test results, interpret data appropriately, and make diagnostic suggestions based on those results.
- Explain to the patient test results and expected health outcomes.
- Prescribe medication or therapy with a physician overseeing as part of a larger health plan.
- Work closely with medical staff to formulate a full, comprehensive treatment plan.
- Administer injections and other medical procedures.
- Assess patient health through interviews and physical examinations.
- Provide counsel and medical treatment to patients.
- Educate patients on overall wellness and health maintenance.
- Develop and implement patient management plans.
- Maintain a safe and clean work environment.
- Provide care to Urgent Care and Worker's Comp patients.
Qualifications
Essential Functions:
- Obtain, compile, and record patient medical data, including health history, progress notes and results of physical examination.
- Discharge planning for patients, writing prescriptions, explaining discharge instructions, confirming follow-up appointments.
- Monitor, record and report systems or changes in patient’s condition.
- Interpret diagnostic test results for deviations from normal.
- Order and/or conduct diagnostic and laboratory tests.
- Maintain accurate detailed reports and records regarding patient’s treatment, history, and plan of care.
- Consult and coordinate with healthcare team members to assess, plan, implement or evaluate the patient’s care plan.
- Collaborate with Medical Director regarding treatment and continuing care when necessary.

- Maintain a neat and orderly work environment, demonstrates the ability to organize and prioritize work during heavy workload situations to expedite patient care.

- Perform quality work within deadlines with or without direct supervision.
- Work independently while understanding the need to communicate and coordinate efforts with other employees and organization.
Minimum Requirements (education, experience, special skills and licenses or certifications required):
- California unrestricted licensed as Family Nurse Practitioner
- Strong organizational skills
- Ability to work in the Electronic Medical record system
- Ability to work independently
