- Valid IL license, CPR Certification.
- Education and/or Experience
- Graduate of qualified School of Nursing; one to two years office nursing experience preferred; or equivalent combination of education and experience.

PRIMARY JOB DUTIES
- Work as a liaison between the patient and the Provider regarding progress and overall plan of care.
- Be knowledgeable and maintain contacts with community resources for equipment and resources that might be necessary for the patient’s on-going care.
- Develop a plan of care, along with the Primary Provider by reviewing medications and other pertinent clinical information to make sure that all information is accurate.
- Educate patient to make sure the patient (and family when necessary) understand the diagnosis and overall plan of care.
- Follow up with the patient to make sure they are meeting goals and following the plan of care.
- Link patient with necessary follow up referrals and appointments.
- Be knowledgeable of financial constraints and understand the pre-cert / prior authorization process.
- Run reports and gather information as directed and needed by the Providers.
- Acts as a patient coach to patients including vulnerable patient populations by offering self-management support and care coordination.
- Other duties may be assigned.
