- Certificates, Licenses, Registrations
- Valid Illinois nursing license with Nephrology certification preferred; CPR Certification.
- Education and/or Experience
- Graduate of qualified School of Nursing; a minimum of three years nursing experience, preferably in Nephrology. Minimum of one-year experience in dialysis is required. At least three months experience in peritoneal dialysis is highly desirable. Additional experience in transplantation is recognized.

PRIMARY JOB DUTIES
- Provides a clean and safe environment for patients, visitors, and staff.
- Assists in the development, execution, and enforcement of policies relative to the protection of patients’ personal and property rights and the general operation of the facilities.
- Maintains and submits appropriate records and reports as may be required by Federal and State agencies and fiscal intermediaries.
- Tracks and documents patient information, which will be used in the unit’s CQI program.
- Serves on the CQI Committee, CKD Committee (if one exists), Patient Care Committee, Risk Management Committee, Infection Control Committee and any other designated patient care committees.
- Manages patient training activities with on-going evaluation of patient response to training and provides feedback for implementation of curriculum changes as necessary.
- Supports education to CKD patients as established within the PD program.
- Provides nursing call in all aspects for PD patients, to include training and retraining, follow-up clinics and consultation, assistance with PD procedures, maintenance of PD patients in home environment, lab testing, and any additional activities that result in the provision of care to the home PD patient.
- Provides the documentation of PD patient outcomes to the PD Nurse Manager, based on Network guidelines, to be reported through the PD unit’s CQI program.
- Acts as a patient coach to patients including vulnerable patient populations by offering self-management support and care coordination.
