About you: The ideal candidate is self-motivated, a skilled communicator, collaborative, and comfortable in the dynamic atmosphere of a growing organization.
Your responsibilities:
- Provide patient education, patient support, and patient care via a telehealth platform as an extension of the physician's practice
- Serve as a liaison and coordinate information between providers, other clinical staff, and patients
- Coordinate the ongoing delivery and management of patient-centered comprehensive care plans
- Complete and initiate appropriate clinical reminders, consults, and alerts
- Utilize EHR to document all clinical observations, information, updates, and patient education
- Schedule, confirm and coordinate virtual patient encounters
- Demonstrate an ability to listen and communicate with others in a professional and caring manner including sensitivity to individuals
- Demonstrate an ability to set priorities for tasks to work effectively despite interruptions and under minimal supervision
Candidates should embody the following qualifications:
- Licensed Practical Nurse (LPN)
- 1-2 years of experience in a healthcare setting (preferred)
- Interest in Chronic Care Management delivered through a Telehealth platform
- Legally Authorized to Work in the United States
