The Metrocare Services Mental Health and Primary Care Division includes a Care Coordination Program that monitors, manages and coordinates the care and services an individual receives. Care Coordination is the deliberate organization of an individual’s care activities and the sharing of information to everyone critical to the individual’s treatment and overall quality of life. Successful care coordination relies on knowing everyone’s needs before services are conducted and communicating to the appropriate people at the right time to ensure safe and proper care.
Successful care coordination is dependent upon collaboration with external providers and partners, especially when individuals are discharging from a hospital, jail and/or residential treatment facility. To ensure a seamless transition from an inpatient setting to Metrocare occurs, each Care Coordination Team will include a Community Engagement Navigator staff member, solely responsible for ensuring a smooth transition for individuals coming into services from an inpatient facility utilizing an organized, person-centered approach that promotes engagement, support, and understanding. The Community Engagement Navigator works with the discharging entity to identify the date of discharge and works with the clinic care coordination team and clinical providers to ensure the individual receives the right care at the right time.
Community Engagement Navigator is a member of the Care Coordination Team. They will be responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between Parkland, Metrocare and the community. They are responsible for working with adults and families in treatment in a manner that is trauma-informed and responsive to needs, culture, gender, and military status. Community Engagement Navigator position is responsible for applying a holistic approach in service delivery. The Community Engagement Navigator will link individuals served to comprehensive treatment services to address identified needs. The Community Engagement Navigator will provide outreach and promote access to services to those who are participating in mental health and substance use services. The Community Engagement Navigator uses knowledge of diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects and benefits to provide services in a person-centered, supportive, community-based environment.
The Parkland Community Engagement Navigator will receive referrals from the Parkland Liaisons who will notify the Parkland Community Engagement Navigator to engage the individual at the discharging hospital and coordinate referrals with RN Manager and Center’s Clinical Manager to ensure continuity of care.
