Metropolitan Nephrology Associates, a medium-sized private practice located in the Washington DC area, is looking for a full-time advanced practice provider (Physician Assistant or Nurse Practitioner) to join our innovative medical team. Our team believes that patients living with kidney disease deserve the best care and through our value-based and patient-centered care approach, we focus on improving patient outcomes and as well as quality of life for our patients with all forms of kidney disease.
Job Responsibilities:
Functions as an independent healthcare provider for the department
Works collaboratively within a multidisciplinary team including physicians, advanced practice providers (APPs), dialysis, and clinic staff
Responsible for providing dialysis care to patients with end-stage renal disease and general outpatient care to patient with acute kidney injury, chronic kidney disease, hypertension, fluid and electrolyte disorders, and kidney transplant recipients
Provides in-person and/or telehealth follow-up care to patients according to established clinical practice guidelines
Provides monitoring and continuity of care between physician visits
Instructs patients and families regarding treatment plan, medications, and comprehensive disease-specific education to patients and families
Job Type: Full-time
Pay: $104,267.00 - $111,221.00 per year
