Description

The Senior Social Worker-PHP is an essential component of the interdisciplinary treatment team, providing clinical care for patients including assessment and treatment planning, group and individual therapy, crisis intervention and disposition planning. The Senior Social Worker assists with onboarding and mentorship of Clinical Social Workers and support the Social Work Supervisor in administrative and operational tasks.

The BWFH Partial Hospital Programs provide assessment, stabilization, and individual and group treatment for patients with psychiatric disorders. The primary goal of the program is to provide guidance, self-empowerment, and healthy living techniques to promote stabilization. The PHPs are part of a continuum encompassing inpatient and outpatient levels of treatment that aim to provide early intervention, treatment and ability to refer internally ensuring continuity of care. The program combines psychopharmacologic treatment with individual and group psychotherapy to allow patients to experience evidence-based treatment at all stages of their recovery.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

* Provides direct patient care including, but not limited to, initial psychosocial assessment and triage of patients, orientation to the program, individual and group treatment, case management to patients and their families, discharge planning, care coordination with outside agencies and patients' families / supports, assessment of cultural issues.

* Provides caregiver/family support and psychoeducation re: mental illness and substance use disorders. Advocates on behalf of patients and families to gain access to services and resources. Refers patients to other providers as necessary.

* Coordinates family/team meetings, as needed and when appropriate. Provides psychosocial consultation on patient care planning and patient/family management and community.

* Assessment for abuse/neglect; files mandated reports as indicated.

* Assessment for intimate partner violence and participates in safety planning for IPV as warranted.

* Identifies psychosocial, financial, legal, or substance use that impact patient care management; collaborates with treatment team to facilitate patient care process.

* Completes documentation (daily progress notes, discharge planning, group notes, billing and other documentation, verification of treatment letters) in accordance with established timelines and standards.

* Works effectively as part of the interdisciplinary health care team, communicating regularly with the team and other members on cases and as issues arise.

* Provide clinical coverage for affiliated programs in the Department of Psychiatry as requested by the supervisor.

* Participate in research or quality improvement projects as requested by the supervisor

* Facilitates the appropriate and efficient use of hospital and community resources.

* Participates in formal and informal clinical case reviews, clinical supervision, and educational seminars.

* Demonstrates a commitment to his/her professional growth and competency in order to enhance patient care and job performance.

* Contributes to the efficient operation of the program, the hospital and, where appropriate, the clinical department through team work, cooperation and participation in problem solving activities.

* Vacation coverage to other Psychiatry/Addictions areas as necessary for safe patient care.

* Attendance at all staff and treatment team meetings.

SENIOR SOCIAL WORKER RESPONSIBILITIES:

* Provides teaching and peer support to PHP interns, trainees and junior staff.

* Provides administrative and operational support to Social Work Supervisor and Manager.

* Conducts review of clinical information provided for new intakes and presents this information in morning rounds.

* Periodic review of group content with focus on maintaining culturally pertinent, evidence-based group treatment.

* Observation of groups facilitated by interns, trainees and junior staff as warranted.

* Co-lead on QI projects.

* Ongoing assessment and identification of community resources.

* Other duties as assigned.

Qualifications

* Masters Degree in Social Work from an accredited program

* LICSW with current professional licensure in Massachusetts.

* Minimum two years' experience as a licensed Behavioral Health clinician in a relevant setting

* Adheres to all hospital-wide standards for professional behavior, as well as cultural sensitivity and age specific competencies

* Experience in the field of substance use and psychiatric disorders

* Clinical experience in individual and group psychotherapeutic and behavioral treatment modalities

* Experience in the integration of medication-based treatment for substance use disorders.

* Skills for assessment, treatment, and crisis intervention

* Ability to communicate effectively orally and in writing

* Interest in outcome measures, clinical research, teaching, patient safety, and quality improvement initiatives

* Ability to use diagnostic criteria based on the DSM 5 to formulate a diagnostic impression and determine level of functioning

* Time management and ability to manage multiple priorities and take initiative

* Strong team player

* Ability to manage confidential information

* Knowledge of community agencies/resources. Ability to advocate/negotiate systems for/with patients.
