JOB DESCRIPTION
Position: PATH Outreach Worker
Classification: Full Time
FLSA: Non-Exempt
Reports To: Executive Director
Supervisory Responsibilities: None
Summary:
The PATH Outreach Worker will identify and build rapport with homeless and/or at risk of homeless
individuals with a serious mental health diagnosis. Workers will engage with eligible individuals and develop the critical relationships necessary for supporting transition to housing and/or needed behavioral health services.
Responsibilities:
Client Services
- Coordinate intake and individualized needs assessment for all eligible individuals
- Develop individualized service plans in partnership with clients.
- Conduct outreach activities in all places homeless individuals congregate.
- Provide all services in a non-judgmental manner.
- Crisis intervention as necessary
- Monitor and evaluate client’s progress through service plan. Modify as necessary
- Provide information, referrals, linkages/introductions, and advocacy.
- Assist clients in procuring necessary documents to receive services/housing
- Identify appropriate permanent housing options
- Assist clients in applications to appropriate programs including potential housing
Contract Services
- Maintain a thorough understand of grant parameters
- Maintain client related data tracking systems including case notes/HMIS entries
- Prepare case-related reports, generate periodic reporting
- Maintain complex and accurate documentation of activities, goals, achievements
- Complete and document follow-up and retention services.
Community Services
- Build strong relationships with community partners to assist clients in achieving goals
- Respond to community requests for outreach intervention
- Network with other agencies, coalitions, and organizations
- Attend local community partner meetings, and all staff meetings and trainings.
Qualifications:
- Minimum Bachelor’s Degree.
- Two years in street outreach
- Computer skills; Microsoft Office, HMIS experience a plus.
- Ability to work independently and as part of a team.
- Strong written and verbal communication skills.
- Exercise mature judgment. Demonstrated good problem solving and conflict resolution skills.
- Manage a high pressure environment, maintain confidentiality, and regular attendance.
- Demonstrate ability to work with diverse communities.
This position requires: Employment Eligibility Verification (eVerify), reliable transportation, and successful completion of background screening.
Physical Requirements:
- Time spent: sitting, standing, walking, driving, bending, stooping, kneeling
- Ability to lift and carry 25lbs.
- Operate a computer keyboard, firm/strong grasping, and repetitive hand control.
- Ability to complete forms in personal handwriting
- Makes and receives phone calls
- Ability to use copier, fax machine, smart phone and other portable devices.
