JOB SUMMARY/GENERAL DESCRIPTION OF PURPOSE:

The person in this position is responsible for providing referral and support services to people in the service area who are homeless, at risk of becoming homeless, or have the potential for reoccurrence of homelessness.  Assists in coordinating services with other shelters or agencies.  Obtains and maintains records and statistics, and compiles information as required.  Attends meetings on behalf of the homeless shelter as needed.  The Case Manager works as a member of the Homeless Shelter team in an effort to best meet the needs of shelter guests and works to contribute to the community’s prevention of homelessness. 


REQUIRED QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.


EDUCATION/CERTIFICATION: Bachelor's degree in psychology or a closely related field, such as mental health, social work, human services, or behavioral health, or case management experience, is necessary to qualify for billing section 13 services.


REQUIRED EXPERIENCE: Experience working with people with mental illness and substance abuse challenges, basic knowledge of homelessness and its causes, ability to make a commitment to the Mid-Maine Homeless Shelter’s mission and organizational values.


SKILLS/ABILITIES:   Demonstrated effective administrative, organizational management and planning skills; ability to comprehend and interpret rules, regulations, and procedures; excellent computer, written and oral communication, and presentation skills; strong interpersonal and critical thinking skills. Ability to maintain confidentiality, ability to multi-task and work independently a must; This is a Monday-Friday Schedule full time position. 



PRIMARY JOB DUTIES

  • Provides case management services including assessment and recommendations, referrals to additional needed services, and individual counseling, including but not limited to area Landlord Associations, Mental Health Providers, Community Action Programs, Police Departments, Municipal Agencies, Employment Services, Health Care Facilities, Housing Programs, Veteran Affairs, and Family Violence Projects, etc.
  • Complete development of goals and plans with families, providing advocacy, monthly plan monitoring, and discharge planning for current and former guests up to 90 days after discharge from the shelter. 
  • Reviews and maintains records and statistics using the Homeless Management Information System (HMIS) to comply with funding sources and program requirements.   Reconciles and assures all information is up to date on a daily basis.
  • Completes and submits all reports and requests required to receive state and federal funding.
  • Connect with guests to make them feel welcomed, safe, and respected.
  • Interviews shelter guests and makes referrals for those in need of emergency housing as soon as possible after guests arrive at the Shelter.
  • Provides initial assessment and Plan of Care within three working days of guests' arrival at the Shelter.
  • Completes intake process with guests and assigns beds.  Obtains and documents all appropriate information provided by the guests relevant to case management services for the guests.   
  • Seventy-five percent of all scheduled working hours in a week must involve face-to-face appointments with guests.    
  • Update the list of current guests on the resource sheet on a daily basis.

Salary range $20-$23 per hour with additional productivity bonus
