Job Title: Case Manager
Duties:
- Conduct assessments and evaluations of patients to determine their needs and develop individualized care plans
- Collaborate with healthcare professionals, including doctors, nurses, and social workers, to coordinate patient care
- Monitor and evaluate patient progress, making adjustments to care plans as necessary
- Provide counseling and support to patients and their families to address emotional and psychological needs
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure they receive appropriate medical treatment and services
- Maintain accurate and up-to-date documentation of patient interactions and progress
Requirements:
- Bachelor's degree in social work, psychology, or a related field (Master's degree preferred)
- Previous experience in case management or a related role
- Knowledge of healthcare systems and resources
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Excellent organizational skills and attention to detail
- Familiarity with electronic medical records systems
Job Type: Contract
Pay: $75.00 - $80.00 per hour
