Summary:
The ideal candidate will have a strong background in social work, excellent communication skills, and a commitment to improving the lives of individuals and families in need. As a Social Worker, you will assess clients' needs, develop care plans, provide counseling, and connect clients with vital community resources.
Midland Cares Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Employment Status: Full-Time| Monday Friday | 8am to 5pm.
Pay range beginning at $54,000 /year (pay based on experience)
$3,000 Sign-On Bonus

Qualifications
Required: Current unrestricted State of Kansas license to practice as a Licensed Master Social Worker. Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling. Must possess valid Kansas Drivers License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organizations insurability standard as defined by Midland policy.
