The Social Worker will plan, organize, and implement social work services to patients and their families to assist them in meeting social and emotional problems arising in connection with an illness or hospitalization. As a member of the care coordination team, the social worker is a member of the interdisciplinary healthcare team and facilitates safe transitions from the hospital to post-acute care.
Qualifications
- Master's degree in Social Work
- Licensed Clinical Social Worker in the state of CT
- Minimum of one year of casework experience highly preferred
- Acute care experience desired
- Excellent interpersonal and communications skills
