Salary: $50,000 - $55,000 per year

Schedule: Monday to Friday, 9:00 AM - 3:00 PM

Location: 6018 SW 18th St C11, Boca Raton, FL 33433

Travel: 50% local travel required

Job Description

Mind and Mobility is seeking a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) to join our team as a Community Liaison. In this role, you will act as a key point of contact for clients, caregivers, and community partners to ensure high-quality care and service delivery. Key Responsibilities

• Conduct home consultations and safety evaluations
• Perform monthly quality assurance (QA) calls
• Conduct employee supervisory visits
• Provide on-the-job training and competency evaluations for caregivers
• Conduct start-of-care processes for new clients and annual client reassessments
• Monitor and assess client condition changes to ensure appropriate care
• Participate in community events and marketing initiatives as needed
  
  

Requirements

Qualifications:

• Active LPN or RN license in the state of Florida
• Minimum of 2 year of experience in home care
• Valid driver’s license and willingness to travel locally (50% of the time)
• Strong organizational and communication skills
• Ability to manage multiple tasks efficiently
