The Community Liaison will play a pivotal role in fostering strong partnerships with various healthcare providers and organizations, including outpatient practice groups, higher levels of care, health systems, health plans, and hospitals. The primary objectives include increasing new patient evaluations and enhancing the overall patient referral experience. The CL will collaborate closely with the Referral Management team to facilitate the processing and scheduling of both external and in-house referrals. Responsibilities will also encompass coordinating outgoing referrals and promptly addressing inquiries from in-house clinicians.
Responsibilities:
- Develop and Maintain Partnerships: In coordination with the Market Partnerships Manager, build and sustain strong partnerships with high-value referral sources.
- Liaison Duties: Act as the primary contact between Mindpath Health (MH) and referral source points of contact, including leadership, clinicians, care coordinators, and case managers.
- Communicate Referral Processes: Clearly explain MH’s referral process to referral partners and provide guidance on improving referral outcomes, such as increasing their conversion rate.
- Complete the Referral Loop: Follow up on incomplete referrals and communicate referral outcomes (e.g., patient scheduled, unable to contact patient).
- Address Inquiries: Answer inquiries, provide information, and resolve concerns from referral partners.
- Collaborate with Clinicians: Work closely with in-house clinicians to place existing patients for new services or identify openings for scheduling external or in-house referrals.
- Process Referrals: Handle referrals by making outbound calls to patients and accepting inbound calls.
- Schedule Patients: Schedule patients referred by external sources and in house clinicians.
- Manage Referral Forms: Ensure all referral forms are promptly uploaded to the patient’s chart after scheduling.
- Respond to Clinician Requests: Address in-house clinician questions, requests, and concerns regarding the status of patient referrals.
- Arrange Outgoing Referrals: Coordinate outgoing referrals as requested by in-house clinicians, ensuring considerations of the patient’s insurance coverage, age, and specific service needs.
- Data Management: Maintain accurate and timely data for tracking, reporting, and follow-up on all activities.
- Feedback Collection: Gather feedback from referral sources and patients to identify areas for improvement in the referral process.
- Documentation: Develop and maintain documentation and resources related to referral processes and community engagement strategies.
- Compliance: Ensure all referral activities comply with relevant regulations and organizational policies.
- Perform Additional Duties: Undertake other related duties as assigned or requested, including special projects as needed.
Qualifications:
- BS/BA degree or two years of position-related experience in lieu of a degree.
- Strong presentation skills and the ability to prepare effective presentations.
- Proficiency with Microsoft Office Suite products, including Excel, PowerPoint, Word, and Outlook.
- Ability to effectively interact and coordinate cross-functionally with other departments, including Clinical Affairs, Operations, and RCM.
- Strong organizational skills.
- Experience with medical records systems and Customer Relationship Management (CRM) systems.
- Excellent verbal and written communication skills.
- Ability to perform effectively, maintain a positive attitude, and work well as a team member while working remotely.
