Oversees the coordination of process and performance/quality improvement activities with all Partnership Health Center departments.

EXAMPLE OF DUTIES
Supervises assigned staff including hiring, scheduling, training, and performance management. Lead the performance excellence team. Coordinates quality improvement initiatives and project teams, including planning, conducting, documenting and evaluating activities. Develops and maintains detailed project outlines and work plans for quality improvement projects.Facilitates and participates in coaching sessions, training, webinars and project calls to support quality improvement initiatives including health center assessments, clinical performance indicators and outcomes, work flows and clinical practice transformation.Guides staff in the use of improvement tools such as brainstorming, flow charts, Ishikawa diagrams, Pareto charts, scatter diagrams, run charts and control charts for the purpose of generating ideas, making decisions and implementing change.Researches and develops innovative ideas for improving specific clinic processes or patient health outcomes. Analyzes and improves evidence based clinical guidelines. Responsible for management, monitoring, data interpretation, and reporting of PHC project prioritization system, project queue, improvement projects progress, and status. Responsible for ensuring that core measures, quality indicators, and risk management activities are communicated timely to internal and external audiences. Ensures data is validated and consistent.OTHER DUTIES: Performs related work as required or directed.SUPERVISION RECEIVED: Works under the direction of the Chief Operations Officer or designee. Works in close collaboration with Operations Department and all PHC leadership.SUPERVISION EXERCISED: Responsible for the direct supervision of Performance Excellence staff.WORKING RELATIONSHIPS: Has frequent contact with leadership and employees throughout Partnership Health Center, and the general public seeking medical care. Work contacts are to coordinate efforts and to ensure cooperation and compliance with PHC standards and policies.

SUPPLEMENTAL INFORMATION
Works primarily in an office setting. The work requires occasional medium lifting up to 25 lbs. Requires ability to work flexible hours and to participate in meetings which may require overnight travel. Work may require exposure to patients with communicable diseases.
