Social Service Coordinator

Major function of position is to provide general case management; including intake, and referral services to all residents needing such services. Must be able to work in a “team” environment.

SPECIFIC DUTIES INCLUDE:

• Maintaining up-to-date tenant profiles, tenant interest forms and community resource files. Develop new program ideas based upon the data contained in them.
• Working with community leaders to create and maintain a working rapport in order to develop new programs geared to the elderly throughout the community.
• Assist tenants’ organizations and social club in planning, scheduling, providing space and equipment for classes, clinics, projects, voting, special events, parties, etc. and keep these groups informed of available resources and programs through the written monthly bulletin.
• Solicit on-site delivery of volunteer and other free services for residents, including, but not limited to, visiting homemakers, legal aid, senior companions, meals-on-wheels, etc.
• Identify potential leaders or persons with special skills and solicit their participation in or initiation of various activities.
• Communicate with the families of tenants when appropriate to solicit support for both the tenants and the activities of the community.
• Provide a referral service for various off-site social service and health care agencies.
• Observe the physical and mental status of residents on a continual basis.
• Maintain effective working relationships with associates and the public.
• Serve as liaison with local and county Offices on Aging.
• Works in accordance with the policies and procedures established by the Board of Directors, MIMC and the NJHMFA.

Work experience:

• 2-3 years’ experience in Social Service delivery with senior citizens and non-elderly disabled; some supervisory experience and management responsibilities desirable.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
