We partner with providers, payors, employers, and patients to make adopting healthier diets and lifestyles a reality.
The Program Data Coordinator position plays a pivotal role in successfully implementing and expanding CalAIM Community Supports services. Coordinators will provide essential oversight and daily operations of enrollment, program adherence, invoicing, customer support, and data reporting, ensuring that the services are delivered effectively and aligned with CalAIM's objectives. They will collaborate closely with Registered Dietitians, MCPs, and CBO leadership to ensure services are tailored to meet the unique needs of individuals and communities. The Program Data Coordinator will facilitate enrollment, ensure timely access to services, and actively engage in care coordination efforts. By closely monitoring and evaluating the delivery of community supports services, they can identify areas for improvement, implement quality improvement initiatives, and optimize the impact of these services on the target population. Additionally, the Program Data Coordinator will serve as a vital point of contact for individuals, providing support and guidance to promote their engagement and utilization of the CS services. Their expertise in project-based work and dedication to patient-centered care will contribute significantly to increasing the reach of the CalAIM CS services, ultimately improving health outcomes, and enhancing the well-being of individuals in need. This role must be linguistically appropriate to serve the diverse needs throughout California, by providing information and support in a manner that promotes understanding and inclusivity no matter the native language or education level of participants. The Program Data Coordinator helps to deliver high-quality care, enhance patient experiences, and is directly responsible for the overall success of the CalAIM program at ModifyHealth.
Work Setting and Schedule
Expected hours weekly: Up to 40 hours, including some evening and weekend availability.
Responsibilities
· Coordinate and process enrollment for individuals participating in payer programs, ensuring accuracy and compliance with program guidelines and regulations per health plan
· Schedule nutritional counseling sessions based on availability and preferences.
· Account for food allergies and preferences, creating personalized meal plans based on RDN guidance.
· Serve as a primary point of contact for health plan members, healthcare providers, and other stakeholders, providing outstanding customer care and support via phone, email, and other communication channels.
· Handle inquiries and resolve enrollment, eligibility, benefits, and services issues, demonstrating empathy and professionalism in all interactions.
· Collaborate with internal teams and external partners to ensure smooth implementation of program initiatives, including timely invoicing and payment processing.
· Maintain detailed records and documentation, track data, and generate reports to support program analysis and quality improvement initiatives.
· Communicate linguistically appropriately, considering cultural, language, and educational backgrounds, as well as considering factors such as housing status and patient ability levels
· Maintain performance KPI dashboard with updates daily.
· Acquire data from primary or secondary data sources and maintain databases/data systems
· Locate and define new process improvement opportunities
Requirements
- Fluency in English and Spanish (required)
- Microsoft Office product expertise required (Excel, Word, Powerpoint, Outlook)
- Strong analytical skills with the ability to collect, organize, analyze, and disseminate significant amounts of information with attention to detail and accuracy
