Care Finders Total Care, a Modivcare Personal Care Service, is looking for a Care Coordinator to schedule patient visits with a caregiver, document communication between patients and caregivers, and ensure branch compliance with patient-related documentation requirements. You will report to the Area Director or local Branch Manager.

- Assign field staff schedules and confirm assignments with employees and patients.
- Speak with patients or patient families regarding schedules, call-offs, cancelations, and appointments
- Document client data and visits in the scheduling system
- Be the backup to the Intake department for receiving and coordinating home care service requests

- High School Diploma or GED
- 1 years of home healthcare experience
- 2 years of customer service experience
- Electronic Visit Verification (EVV) system experience
- Bilingual in English & Spanish required

Pay: $17.00-$19.00/Hourly
Schedule: M-F 8:30AM-5:00PM
