Job Posting for Patient care Coordinator / Care Navigator at Moments Hospice
We are currently looking for Patient Care Coordinators to join our Team.
1 position to work M-F 08:30am-5:00pm
1 position to work M-F 08:30am-5:00pm - Bilingual English and Spanish
1 position to work M-F 07:00am-03:30pm
As a Moments Hospice intake coordinator you will be responsible for working in all aspects of the patient intake process for Hospice, Palliative Care and Connections programs, establishing and maintaining positive relationships with customers and referral sources, collecting data on patients, and assisting with the insurance verification and authorization process.
Must be able to work in a fast paced environment and Have excellent communication skills.
Job Description:
- Answer phone calls and incoming patient messages.
- Direct phone calls.
- Responsible for medical records in Forcura (uploading into HCHB).
- Conduct or assist with special projects as requested by the administrator and other Moments Hospice leadership
- Must be able to work in a fast paced environment and Have excellent communication skills.
- Required skills and qualifications:
- High school diploma or GED; Associate or Bachelor's degree preferred
- Two years of office experience, preferably in a hospice agency setting
- Proficiency with computers and software applications, including Microsoft Office
- EMR software experience highly preferred
- Hospice experience preferred
- HCHB experience preferred
