POSITION SUMMARY: The Care Coordinator, LPN assists the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN in the management and leadership of a specific nursing unit.
JOB DUTIES: This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care. He/she also assists in the direction and implementation of the nursing process. This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection; assist with the development of care plan, and implementation of care plan and revision of plan. The Care Coordinator, LPN coordinates the management of the care of residents through a well defined care delivery system.
QUALIFICATIONS: Must have current registration, as a Licensed Practical Nurse, in the State of Minnesota. Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider. Must possess the ability to communicate verbally and in writing when coordinating the care of the residents. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment, after criminal background study, per guidelines of Minnesota Department of Human Services.
