Position Overview: The Caregiver Specialist conducts care planning assessments to identify problems and to assist the individuals to provide solutions. The Caregiver Specialist provides caregiver support, information and outreach for the Caregiver Support Program including, needs assessment, caregiver counseling, and identifying and assisting in developing resources to meet the needs of the elder of caregiver. This position maintains high visibility in the community and maintains multiple contacts for MHCC with healthcare institutions.
Responsibilities include: Conduct care needs and planning assessments to identify problems and to assist the individuals to provide solutions and next steps. Provide caregiver support, information, referral and outreach through home visits and/or telephone contact. Provide caregivers with a person-centered approach that ensures their goals preferences are honored. Establish and conduct outreach activities to educate community partners; such as hospital, rehabilitations and nursing facilities, Councils on Aging and other community agencies and providers to raise awareness of program and generate mutual referrals. Provide short-term individual counseling; facilitate support groups and specialized caregiver trainings in the community as needed.
Qualifications: The ideal candidate will have excellent:
Bachelor’s degree in relevant field and two years’ experience providing case management supports to elders and/or persons with disabilities are required. Successful candidate will have reliable transportation with a willingness to frequently travel throughout assigned areas.
Compensation and Benefits:
