The Social Worker shall interview the resident and/or family within 14 before or after admission to identify any social work needs or problems and to take a social history that includes family constellation, education, occupational background, adjustment and level of functioning, interest, support systems and observations.
The social worker shall provide individual counseling for resident and families as needs are identified, the social worker provides leadership in group activities with families or residents and keep records of attendance and outcome.
The social worker shall offer information and help to each resident and family on obtaining financial assistance and on the meaning of administrative forms and releases to be signed by the resident or family
The Social Worker shall coordinate the facility’s outreach program or services to families of residents. The Social Worker shall coordinate the discharge services for residents.
The social worker shell perform advocacy services on behalf of the residents to ensure that concrete needs are met, such as clothing, laundry, and the resident's personal needs allowance if one is maintained.
A social worker shall help residents and families identify and gain access to community services, using resource materials and a knowledge of the resident's needs and abilities.
The social worker shall keep records of contacts made and provide clinical social work to residents as needed and to families if related to issues that directly affect the resident.
The Director of Social Services shall supervise any part time social worker and set parameters of work required to be completed.
The Director shall assure timely completion of paperwork and oversee the quality of the work of the part lime social worker.
When possible, the social worker or director participates in preadmission planning with residents and families prior to admission.
Social workers work during the week but are available at night and on weekends during emergencies.
Whenever possible the social worker or director monitor family involvement and monitors visiting patterns in order to provide outreach or services where patterns have changed.
Provides leadership on the interdisciplinary team and leads the meeting. Documents the meeting and provides staff with ideas and ways of managing issues or resolving problems.
Provides sensitivity and rights training to staff of all departments. Provides specialized training upon request or upon identified needs. Keeps accurate documentation of same,
