SUMMARY

The Mobile Integrated Health ("MIH") Coordinator will lead the Mobile Integrated Health Program within the Department of Public Safety and work collaboratively with multiple offices of the Montgomery County Department of Health & Human Services to address the issues of complex patients who use the Emergency Department. The program uses mobile resources to deliver care and services to patients in an out-of-hospital environment in coordination with healthcare facilities or other healthcare providers. This program additionally focuses on substance use disorder patients to prevent unnecessary emergency department visits and establishing a clear referral pathway from the field to long-term rehabilitative care. The MIH Coordinator shall be a certified PA Dept. of Health paramedic, approved for unrestricted medical command authorization by the DPS Chief Medical Officer, and be responsible for coordinating identified patient cohorts for follow up by the multi-disciplinary response team.

The MIH Coordinator conducts remote or in-person health assessments via telephone or other means and works directly with the Office of Drug and Alcohol, the Office of Public Health and the District Attorney’s Office to ensure that patients receive the right care, in the right place and at the right time via the Montgomery County Diversion Center. The MIH Coordinator is assigned to the EMS Training Institute for daily coordination and support. Additional responsibilities include direct support and direct delivery of Community Health Education programs such as CPR, Naloxone administration and Stop-the-Bleed initiatives. In addition, the MIH Coordinator will be responsible for the execution of special projects as assigned by the Department of Public Safety and the Office of the Medical Director which involve the provision of medical care in non-traditional roles including support for COVID-19 mitigation efforts.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Serve as the Medical Quality Assurance official of the MIH Program and be certified by the Commonwealth of PA as a paramedic or higher;
  • Provide oversight of the MIH paramedics including all aspects of patient care;
  • Provide clear, concise instructions to the MIH Paramedics, which are within their scope of practice;
  • Assist the CMO and /or Regional Medical Director with the coordination and implementation of a regional MIH Medical Control system. Collect and review data regarding the quality of MIH team’s patient care and report to the regional Medical Advisory Committee (MAC);
  • Coordinate with other members of the Multi-Disciplinary Response Team; Actively outreach and enroll substance use disorder patients in the MIH Follow-Up Program and assist H&HS case managers in evaluating and enrolling potential new patients prior to discharge from long-term treatment;
  • Responsible for developing and maintaining relationships with the Montgomery County Office of Managed Care management team for developing processes to facilitate enrollment of substance use disorder patients into the MIH Program. Meet daily with other members of the multi-disciplinary response team as well as case management officials to discuss enrolled patients and any needed follow-up visits for the day.
  • Coordinator shall be culturally competent and trained in providing crisis assistance. For behavioral health issues, the MIH Coordinator shall work in conjunction with the MIH Paramedics and Office of Behavior Health’s Behavioral Health Response Team ("BHRT") and facilitate the following:
    • post-acute care follow-up and treatment compliance;
    • screen patients for depression and substance use disorders;
    • perform outpatient medication reconciliation;
    • develop options to manage escalating needs in community setting;
    • ensure patients are connected to primary care, appropriate behavioral and specialty care;
    • Check Medicaid recertification date and assist patients with process to maintain coverage.
  • Respond on an as-available, as-needed basis to 9-1-1 emergency calls as dispatched by the Montgomery County Communications Center;
  • On an as-needed basis, respond to scenes support the operations of Montgomery County’s special operations teams such as USAR and HAZ-MAT
  • Coordinates special projects as needed and assigned by DPS;
  • Provide services including testing, public outreach, and direct medical care for persons under investigation or with suspected or confirmed COVID-19;
  • Provide emergency and non-emergency clinical and administrative services as an MIH paramedic in a variety of settings;
  • Note that the individual performing this job may reasonably anticipate coming into contact with human blood, oropharyngeal secretions, and other potentially infectious materials. Individuals in this position are required to exercise universal precautions, use personal protective equipment including N-95 or greater respirators, face shields, gowns, and other devices. They must learn, understand, and comply with the policies concerning infection control procedures and processes.

QUALIFICATION REQUIREMENTS

  • Experience: Currently certified PA DOH Paramedic, PHRN, or RN with a minimum of 5 years of clinical experience providing prehospital emergency care or significant expertise in the arena of provision of mobile integrated health care;
  • Proficiency with using electronic medical record system / patient care reporting (PCR) systems;
  • Proficiency with MS Word / Excel / PowerPoint;
  • PA Child Abuse Clearance;
  • PASP Criminal Background Check with renewal;
  • Valid driver’s license, reliable transportation, and PA EMSVO;
  • High school diploma or equivalent. 2 years of college preferred. Preference will be given to those with Bachelor’s degrees. Strong preference will be given those with formal public health training;
  • Ability to work morning, afternoon, evening, overnight, and weekend hours as necessary;
  • Ability to obtain and maintain unrestricted medical command privileges from the DPS Chief Medical Officer or his or her designee;
  • Ability to safely operate vehicles including emergency vehicles such as a large SUV or an ambulance in non-emergency and emergency modes of operation.
  • Knowledge of the design and operation of the PA Medical Command System;
  • Trained in and thoroughly familiar with EMS levels of training and responsibilities as well as the responsibilities of a medical control physician.
  • Excellent communication and organization skills including ability to work as part of a team;
  • Maintain current CPR & ACLS certification;
  • Ability to carry tasks assigned to completion;
  • Strong professional ethics;
  • Ability to quickly react to changing situations to preserve the safety of him/herself, other responders, and the general public;
  • Ability to establish strategic priorities while remaining focused on daily operations;
  • Prefer candidates currently working or living in Montgomery County or a directly adjacent county.

This position requires availability 24 hours a day, seven days a week, 365 days a year. This is an Essential Employee position. Essential employees are obligated to report to work as scheduled or remain at work during emergencies.
