As a Coordinator at our home health agency, you will serve as a central point of contact for clients, caregivers, and healthcare professionals, facilitating the delivery of high-quality care to individuals in their homes. You will be responsible for coordinating care services, scheduling appointments, and ensuring effective communication among all stakeholders. Your attention to detail, organizational skills, and dedication to client satisfaction will be essential in supporting the agency's mission of providing compassionate and comprehensive care.
Responsibilities:
- Coordinate the intake process for new clients, including gathering necessary information, verifying insurance coverage, and conducting initial assessments.
- Schedule appointments for clients with caregivers, nurses, therapists, and other healthcare providers, ensuring efficient coordination of services and timely delivery of care.
- Communicate with clients and their families to provide information, answer questions, and address concerns related to care services, schedules, and billing.
- Maintain accurate and up-to-date client records, including demographic information, care plans, and documentation of services provided.
- Collaborate with healthcare professionals to develop and implement comprehensive care plans tailored to each client's needs and preferences.
- Monitor the delivery of care to clients, including conducting regular check-ins and follow-up calls to assess satisfaction and address any issues or concerns.
- Coordinate with insurance companies and other third-party payers to verify coverage, obtain authorizations, and facilitate billing and reimbursement processes.
- Assist with staffing coordination, including scheduling and assigning caregivers to clients based on their skills, availability, and client needs.
- Provide administrative support to the clinical team, including filing, data entry, and preparation of reports as needed.
- Participate in quality improvement activities, including audits, performance improvement projects, and outcome measurements, to ensure compliance with regulatory standards and promote continuous quality improvement.
Requirements:
- Bachelor's degree in healthcare administration, social work, nursing, or a related field preferred.
- Previous experience in a healthcare or home health agency setting preferred.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills, with the ability to interact effectively with clients, caregivers, and healthcare professionals.
- Proficiency in computer skills, including electronic health records (EHR) such as Vesta, HHAeXchange, Availity, TMHP and Microsoft Office Suite.
- Ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
- Knowledge of insurance verification, authorization processes, and billing procedures preferred.
- Commitment to upholding ethical standards and maintaining confidentiality of client information.
- Valid driver's license and reliable transportation required.
Working Conditions:
- This position operates in an office environment within a home health agency.
- Regular hours are typically Monday to Friday from 8:30 AM to 5:00 PM Central Time.
