SUMMARY

Responsible for intake and follow-up of individuals participating in the Promise Fund Grant Proposal.

PRIMARY FUNCTIONS

· Schedules and manages screening for grant participants.

· Monitors and organizes high risk individuals for follow-up.

· Communicates grant progress regarding deliverables to MLHC management team.

· Develops a resource directory for individuals in high-risk category in conjunction with

· MLHC management

· Maintains logs of contacts for all individuals screened and/or in follow-up program.

· Coordinates resource directory of services and programs available to promise participants.

· Maintains directory of campus mentors available to Promise participants.

· Performs all other duties as assigned.

· Procure educations and community events – (highly important to recruit participants!!!)

· Participant’s recruitment at educational and/or Community events

· Maintain treatment schedules and communication with providers.

· Determination of eligibility for medical coverage and program onboarding.

· Provide transportation and translation during medical screenings.

QUALIFICATIONS (EDUCATION and/or EXPERIENCE)

· Knowledgeable about local community resources

· Superior communication skills

· Able to work independently with minimal supervision.

· Sensitive to women's health needs and provide a role model for high standards of health care.

· Skilled in community outreach

· Skilled in collecting data and maintaining statistics for grant reporting.

LANGUAGE SKILLS

· Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

· Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

REASONING ABILITY

· Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

· Ability to deal with problems involving several concrete variables in standardized situations.
