Summary
  • This position participates in the interdisciplinary team. Provides service to inpatient and outpatients with attention to diabetes treatment modalities and education. This position will explore customer needs and find creative solutions to satisfy those needs.
  • This position works under the supervision of the Vice President Medical Services and is employed by Mosaic Health System
Duties
  • Identifies and utilizes available and appropriate community services to enhance Diabetes Care services based upon customer needs.
  • Conducts patient assessments, interventions, discharge planning if indicated, outcomes and goals in the medical record in compliance with Diabetes Care and regulatory guidelines.
  • Assists in the development of Standards of Care, Forms and other Diabetes Care tools.
  • Educates and acts as a resource to providers, nursing and ancillary department caregivers.
  • Provides group and individual diabetes patient education sessions and documents to meet with organization, Diabetes Care and other regulatory agency requirements.
Qualifications
  • Graduate of school of nursing or dietician. Registered nurse (Master’s preferred) or Registered Dietitian with previous experience focused in diabetes care.
  • Current Mo. State license or Registry. Certification as a Certified Diabetes Educator or must obtain certification within 2 year from date of hire.
  • Minimum of three (3) years of previous experience in a hospital setting. Experience in treatment and education principles with diabetes patients preferred.

Salary : $60,008 - $90,022
