Details Patient Liaison Wellness Full Time Status Day Shift Summary In response to current isolation procedures and visitation policies, this position will serve by being a link between the patient and their loved ones. This position will assist the isolated COVID-19 patients with social connection and specific non-clinical needs. This position will appropriately refer and connect the patient to other team members as needs arise. This position will acknowledge the importance of infection control and physical distancing measures while helping patients and their loved ones manage their social hospital experiences in the setting of COVID-19 with effective communication. This position will be physically present in the patients' rooms in the inpatient setting on COVID-19 units. This position will ascertain what subjects are of most importance to this population and develop relationships to aid in our value statement that the needs of the patient come first. This position will work under the supervision of the Director of Organizational Development and Well-being and is employed by Mosaic Health Systems. Duties Build relationships with patients in the COVID-19 inpatient setting Maintain active engagement between patients and their loved ones by leveraging technology to schedule and support Zoom calls Prioritize timely and clear communication to the patient’s loved ones Help providers manage expectations of patients and patient’s loved ones Build confidence, reduce anxiety and vulnerability through patient and loved one support Deliver patient meals into the patient rooms when delivered to the floor by Dietary Complete tasks to support the patient and the employees in a timely manner, i.e. refill water cups Other duties as assigned Qualifications All required education is a minimum requirement. Higher levels of education are acceptable. High School diploma required. 2 Years experience in a customer service role, demonstrated working knowledge of supportive services and other resources in the area served by the project and demonstrated ability to advocate, organize, problem solve, and provide results for the population served required.
