Schedule :
Standard operating hours are 8 : 00 am 5 : 00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.
Hourly Wage :
$15.50 / hr. to start - $16.50 / hr. after 90 Days

Job Summary :
Scheduling of all in-center and home care Personal Care Aides (PCA) to ensure Participant care activities are aligned with their plan of care.
Provide personal care and assistance to participants in the center, home, or domicile care setting.
Duties / Responsibilities :
- Provides supervision, personal care, and basic housekeeping to Participants in the center, home, or in the community.
- Ensures center and home care PCA operations and environment are within audit standards, including completion of all required logs.
- Will attend all IDT meetings in the absence of the Direct Care Aide Coordinator.
- Communication of in-center and home care PCAs for shifts and work assignments with the approval of the Direct Care Aide Coordinator for schedule changes, PTO or OT.
- Will complete the master schedule, center assignments, home care schedules, and participant scheduling in Alaya Care.
- Assists with the coordination of the participant supply process including coordination of delivery.
- Will monitor schedules to ensure participants services are being completed as care planned.
- Complete necessary documentation and encounters in the EMR in a timely manner.
- Assists in completion of audits, including but not limited to home care PCA documentation.
- Responsible for communicating to all PCAs their schedules and assignments.
- Responsible for reviewing electronic timesheets and verifying all PCA payroll, mileage, and expense reimbursements.
- Participate in on-going training / education as directed.
- In the absence of the Direct Care Aide Coordinator, will complete all initial, six months and significant change assessments in the home to assess participants resources, safety, functionality and living environment.
- Assist in completing initial and annual competencies for all in-center and home care PCAs in the absence of the Direct Care Aide Coordinator.
- Fill-in as a center or home care PCA, if needed, at the direction of the Center Manager and / or Direct Care Aide Coordinator
- Other duties as assigned.

Education and Experience :
- 18 years of age or older
- High School Diploma or equivalent
- One year of experience working with the frail and elderly population
- One year of supervisory experience
- Valid driver’s license
- CPR and first aid certification
Requirements :
- Requires valid State driver’s license with a clean driving record.
- Successful completion of background check, physical, drug screening, TB Screening
Physical Requirements :
- Must be able to move intermittently throughout the workday
- Must be able to lift, push, pull, and move a minimum of 25 pounds
- Must be able to assist with the evacuation of Participants
- This position may encounter
issues / infestations
in the homes of the Participants. Following proper protocols is essential.

Employment Requirements : Successful completion of Criminal History Check, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer’s expense).
Successful completion of post-offer employment drug screening and physical (at employer’s expense). CPR and First Aid Certification, or willing to obtain at
employer’s expense. Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.
This position has full access to Participant, program, and staff records and reports, requiring discretion to protect individuals and the program.
