Scheduler - Home Care and Hospice

Mozaic Home Care and Hospice seeks an experienced Scheduler to schedule clinical visits, provide high level of customer service office support and work in collaboration with the clinical teams.

Essential Responsibilities

  • Able to effectively and efficiently schedule nursing, therapy, and aide visits to meet patient and agency needs.
  • Maintain up to date Task Lists per protocol.
  • Address outstanding and/or unassigned visits daily.
  • Competent computer skills
  • Secure coverage for visits when Call Outs occur.
  • Communicate with clinicians, patients, and family members on a regular basis to ensure complete and accurate scheduling needs are met.
  • Work collaboratively with office teams including clinical supervisors, clinical liaisons, and schedulers.
  • Work in collaboration with the billing team to ensure insurance authorization is obtained for scheduled visits.
  • Provide excellent customer service.
  • Participate in On Call coverage schedule.
  • Answer and appropriately triage agency phone calls.

Minimum Qualifications:

Education: High School Diploma or GED

Experience: 3-5 years in Home Care/Hospice setting or related field


Physical and Mental Requirements:

  1. Ability to sit or stand for long periods of time.
  2. Ability to speak clearly and be understood.
