Description
JOB TITLE: Targeted Case Management (TCM) Case Manager FLSA STATUS: Hourly/Non-Exempt
JOB LOCATION: Social Services – Adult Programs
PRIMARY FUNCTION: Responsible for assessing the consumer’s physical, psychological, and social environment needs with the goal of facilitating personal health, community participation, empowerment, and supporting the consumer’s recovery.
PRIMARY DUTIES:

1. Engage consumer in recovery-oriented, consumer centered care in accordance with professional standards of practice. E
2. Conduct a thorough assessment of the consumer’s needs, including arranging for specialized services, as needed. E
3. Develop and implement the treatment plan in collaboration with the consumer, including but not limited to: counseling and therapy services, use of community resources, vocational training programs, home visits, and other needed services. E
4. Monitor treatment plan and make updates to best meet the consumer’s needs. E
5. Advocate on behalf of the consumer/family for needed services and benefits. Assist in obtaining reimbursement and benefits including, medical assistance, transportation, educations, etc. E
6. Provide education, modeling, and support in the areas of daily living, budgeting, money management, and medication management. E
7. Prepare, complete and submit required items by due dates to include, but not limited to: TCM compliant documentation within 24 hours of the contact, consumer records on database, etc. E
8. Follow legal, organizational and contractual requirements, laws and policies (i.e.; County Contract). E
9. Implement and/or practice the duties of a mandated reporter of suspected child abuse under current policies and procedures, as required. E
10. Establish and maintain good communication and cooperation with all stakeholders. NE
11. Attend all meetings and professional development/training and collaborate (i.e.; BHD), as directed. NE
12. Perform other duties as assigned. NE
Requirements

The employee is regularly required to drive, stand, sit, reach, stoop, bend, and walk. Employee is frequently required to see, talk, or hear. Finger dexterity is also required. Infrequent lifting, including files, will be encountered.
WORKING CONDITIONS:
Work is generally performed in field and office environments, including travel. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.
REPORTS TO: TCM Supervisor
MINIMUM QUALIFICATIONS REQUIRED:
1. Bachelor's degree from an accredited school in social work, psychology, sociology, or like field.
2. One (1) year of experience working with adults who have significant mental health needs.
3. Able to work and communicate frankly and effectively with adults, families, and all team members to implement problem solving strategies, understand issues, and to provide crisis services in an ethical, respectful, responsible manner
4. Demonstrated cultural competency with diverse groups and ability to work with individuals from diverse economic backgrounds in a respectful manner.
5. Excellent communication skills, both oral and written, interpersonal relationship skills, and able to work effectively with others from culturally diverse, linguistic and economic background; both in a team and individual environment. Ability to clearly document all interventions/contacts.
6. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems and ability to adequately implement Plan strategies.
7. Highly developed organizational skills and able to manage multiple priorities.
8. Knowledge and experience with experience in using Microsoft Office suite.
9. Transportation, valid Wisconsin Driver’s license, state minimum auto insurance and must meet La Causa, Inc. driving standards.
10. Able to work flexible hours, including evenings and weekends depending on needs of the youth/family and program needs.
11. Must successfully complete and pass all background checks (including vaccination requirements).

Salary Description
$38,889.61 - $48,228.90
