Job Overview:
The Nurse Clinical Diabetes Educator is responsible for the assessment, implementation, and evaluation of the patients’ status, plan of care, and goal achievement. The perfect candidate is expected to uphold the most ethical and safe processes while keeping the care of the patient as a top priority. The position works closely with the entire multidisciplinary team and provides safe, effective, and culturally competent patient care in accordance with the organizational policies and procedures. This position performs the duties to support the MCT2D Champion (Michigan Collaborative for Type 2 Diabetes) Program.

Responsibilities:
Provides direct nursing care to patients, performs examinations, and assists health care providers with medical procedures.

Completes endocrinology nurse visits for injections.

Documents nursing actions in patients' records using the electronic medical record system.

Assists in coordinating total patient care.

Completes endocrinology DME forms, foot exam forms, sets up osteoporosis infusions, 72 hour fasting glucose tests, and cortisol function testing.

Performs prior authorizations. Assists with planning, maintaining, and evaluating policies and procedures for the clinic.

Performs the duties of the MCT2D (Michigan Collaborative for Type 2 Diabetes) Program Champion which include managing data of patients enrolled in MCT2D program which may include running reports, identifying areas for improvement, reporting back to providers with data and planning for improvement, connecting with patient to ensure understanding of plan, attending monthly MCT2D PO meetings, attending biweekly HCI MCT2D meetings, attending quarterly in person MCT2D meetings, ordering CGM for patient, performing professional CGM application and training and downloading of CGM reports for all endocrinology CGM patients.

Qualifications:
Current/Valid R.N. License with the state of Michigan

Minimum of 2 years experience as a Registered Nurse

Recent professional experience with diabetes population

Obtains CDE certification within 3 years of hire.
