Description
Provides comprehensive health care to patients in collaboration with physicians and other medical staff including medical evaluation, counseling and referrals. Assesses the physical and psychosocial status of patients by means of interview, health history and physical examination and diagnostic tests. Interprets the data, develops and implements therapeutic plan and follows through on a continuum of patient care across the health-illness continuum and life span.
Essential functions include but are not limited to:
- Evaluates and implements patient health care needs.
- Committed to providing quality primary care for patients at outreach clinics.
- Performs routine exams and obtains patient histories.
- Manages patient treatment program based on assessment of needs.
- Promotes and participates in community health education and outreach programs.
- Initiates treatment via prescriptions, diagnostic tests, exams and procedures.
- Review laboratory results and plans actions according to results.
- Performs routine health maintenance activities for new and established patients.
- Coordinates patient care and makes referrals to physicians, RN’s, psychologists, nutritionists, etc. as needed.
- Collaborates and communicates with other nurses, physicians and outside agencies regarding patient care.
- Coordinates patient care and family counseling with public and mental health agencies as appropriate.
- Teaches wellness habits and educates patients on prevention of potential health problems or methods of dealing with present problems, providing information and materials relating to patient health care needs.
- Develops educational programs for medical/and or nursing students and the general public, upon request.
- Embraces the use of the Electronic Medical record as requested by established protocols. Establishes collegiality with other care providers and works with them as a group.
- Submits data for billing on a timely basis as required by Clinic Administrator.
- Performs administrative duties such as writing admission and discharge orders, working with office staff regarding patient problems, scheduling, records and the like.
- Maintains knowledge of current medical practices and skills.
- All of the above activities are performed within the specific, approved protocols that have been agreed upon by the collaborating physician and Mid-Level Provider.
Requirements
Professional Requirements:
- Working knowledge of CPT; HCPC’s; and ICD-10 coding documentation procedures.
- Documentation to support education, license, training, and experience must be provided to support your stated qualifications.
- Maintains regulatory requirements.
- Completes annual education requirements.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled, completes work within designated time.
- Wears identification while on duty.
- Completes in-services and returns in a timely fashion.
- Attends annual review and department in-services, as scheduled.
- Represents the organization in a positive and professional manner.
- Actively participates in performance improvement and continuous quality improvement (CQI) activities.
- Complies with all organizational policies regarding ethical business practices.
- This position is responsible for wholehearted, genuine operations of all aspects of the safety and health program – including compliance with rules and regulations- and for continuously practicing safety and health while performing their duties.
- Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
- Ability to work independently within the various Bay Care Medical Center outreach clinics.
Regulatory Requirements:
- Current certification to practice as an Advance Nurse Practitioner and/or Physician Assistant in the State of Michigan required. Certification as nurse practitioner by the American Nurses Association required for NP.
- One (1) to Two (2) years of experience as an Advance Nurse Practitioner and/or Physician Assistant preferred but not required. An equivalent combination of education and experience from comparable knowledge and abilities may be acquired and will be considered.
- Current BLS, ACLS, PALS, CALS, ATLS certification.
Supervision Received:
General Supervision is received from the clinic physicians. Consultation is available regarding special problems. Generally follows established protocols and collaborative agreement with clinic physician/medical director. Has day to day independence in health care decisions, within the scope of the practice.
Physical Requirements:
- Prolonged walking/standing/sitting.
- Ability to lift and position patients with assistance of other personnel.
- Ability to push patients on gurneys/in wheelchairs.
- Ability to lift/push/pull a minimum of 25 pounds.
