Release Planner

NaphCare is hiring an experienced Release Planner just like you to join our team at the Arizona Department of Corrections - Winslow facility located in Winslow, Arizona. We have opportunities for full-time with great benefits, part-time, PRN, and 13-week temporary contracts available! Pay rates range from $25.50 - $31.87 hr per hour based on type of license, certification, Shift, FT, PRN, temporary contract, site needs, licensure, and experience.

NaphCare has a partnership with NetCE that provides CEU/CME for our staff. NetCE uses a rigorous peer review process to ensure that all activities and content are up-to-date. This service streamlines continuing education for all NaphCare Employees to meet state specific requirements for maintaining licensing.

Position Description:

• Conduct rounds with physicians to meet with patients to discuss placement orders
• Assist patients with a specific plan for medication management
• Include patient in discharge planning
• Assist patients with the identification of a support network, instructions for future care and follow-up
• Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
