  • Provide compassionate and respectful care and support services to help clients address the complex challenges associated with living with HIV.
  • Welcome clients to CARES and assess their needs upon arrival.
  • Respond to client calls with kindness and empathy while ensuring clients are connected to appropriate resources.
  • Ensure clients are able to access all needed support services provided by Nashville CARES, as well as resources in the community.
  • Assist clients with connections to medical, social, community, financial, and other needed services.
  • Assist clients in developing realistic action steps to achieve their goals.
  • Provide support services and referrals to clients who may be socially isolated.
  • Provide transportation to clients as needed to access medical, community, and agency services.
  • Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged with in the community.
  • Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service.
  • Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care.
  • Other duties as assigned.

 

Minimum Qualifications:

  • Bachelor’s degree (preference for Bachelor’s in Social Work or other Human Services) OR Minimum of one year experience in community based social or health care services and a high school diploma or GED.
  • Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology.
  • Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work.
