Program Coordinator- Community Living Services Program
EXEMPT
SUMMARY: Provides administrative and clinical responsibility for the management of a group of apartments/group homes. Supervises House Managers. Ensures waiver and non waiver case management services to assure service identification, delivery and funding authorizations. Applies best practices to ensure clinical and environmental supports are in place to provide quality services that are important to and for the individuals. Establishes and maintains administrative practices for the apartments/group homes. Maintains close and positive relationships with referral /sponsoring agencies as well as regulatory bodies.
REPORTS TO: Director of Community Living
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
The below statements are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties and/or skills required. Other duties may be assigned.
- Provide direct supervision, coaching and feedback to House Managers/Residential Supervisors in the completion of his/her job duties. Prepare payroll for direct reports. Monitor and assess staff performance and complete annual evaluations for House Managers/Residential Supervisors.
- Oversees apartments/homes are maintained in proper condition to assure compliance to regulatory requirements and support for the life-style of people served.
- Assists DDS/DDA in the Individual Service Plan (ISP) process and planning. Oversees implementation of Individual Service Plan (ISP). Complete all required assessments i.e., safety plans, evacuation, voting etc.
- Ensures Residential, medical, and other clinical portion of the ISP are completed for services provided or contracted by NCC. Conduct Person Centered Planning meetings. Develops outcomes that support Quality of Life and represent what is important to and for an individual.
- Ensures staff are trained in all required topics, including but not limited to First Aid, CPR, Individual Rights, Incident Management and Reporting, ISP goal documentation, core competency, DDS/DDA (MD) mandated trainings, OHSHA, and Individual specific needs etc.
- Conducts relevant training as needed. Oversees new staff orientation applicable to the program, site, and individual.
- Serves as a liaison between families, guardians, and external agencies.
- Maintains all pertinent information related to the individual. Coordinates service delivery and case management for services provided or contracted by NCC. Oversees all data collection.
- Oversees waiver authorizations to include identification of waiver services needed as part to the ISP process or changing needs of an individual, assures documentation of waiver services, and secures authorizations prior to expiration.
- Coordinates and collaborates with day program and visits work locations at least monthly. Secures and maintains transportation to and from various locations in the community.
- Administrative supervision of nursing staff and oversight of medical services. Oversee i.e., Health Passports and HMCP. Collaborate with Nurse Manager and have weekly meetings.
- Assist with management of individual funds in accordance with NCC/DDA/DDS policies and procedures.
- Completes quarterly reports and court reports.
- Attends all required meetings, trainings and court appearances.
- On call after normal business hours to address any emergencies and or issues of concerns.
- Prepare for internal/external monitoring, surveys and audits.
- Performs other duties as assigned.
QUALIFICATION REQUIREMENTS: (Required to perform essential functions)

EDUCATION and/or EXPERIENCE:
- B.A. Degree in a Human Services Field with one year of specialized training and one year experience working directly with people with Developmental Disabilities
- Must have at least 5 years experience in Human Services, of which 3 years are supervisory and administrative.
LANGUAGE SKILLS:
- Ability to effectively communicate orally and in written with internal/external vendors/clients, staff and management, and maintain professional relationships. Ability to read and comprehend simple instructions, business correspondence, and memoranda.
REASONING ABILITY:
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
