Job Posting for Community Health Worker at National Health Corps - Central California
Major Duties and Responsibilities
1. Interact with patients and the community about services and programs
2. Support and assist clinical support staff on health programs/services such as vaccination, health screenings preparation, etc.
3. Work collaboratively with outreach to improve patient health outcomes
4. May work with the team to research, collect, and analyze patient health data to better address access and better health outcomes
5. Must be knowledgeable about the towns/locations and health needs
Knowledge Required for the Position
Knowledge of AmeriCorps/National Health Corps member requirements
Skill with Microsoft Office or other software
Additional Notes:
Members will be provided with the Community Health Worker Certification, the Digital Navigation Certification, and (if it applies - must be bilingual) the Medical Interpretation Certification. Members will be offered full-time employment at the health clinic, given that the member graduates the program and performs to their standards.
Job Types: Full-time, Contract
Pay: $27,200.00 per year
