Responsibilities of the Nurse Practitioner - Health Risk Assessments:
- Conduct comprehensive in-home or remote health risk assessments on Medicare Advantage members
- Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
- Educate and counsel patients on identified conditions and screening procedures
- Document findings using the provided iPad
- No continuum of care, prescribing, on-call, or follow-up is required
Requirements of the Nurse Practitioner - Health Risk Assessments:
- Active and unencumbered Nurse Practitioner license
- Minimum availability of 12 hours per week (including one full 8 hour day) is required to qualify
