Job Title: ECM Case Manager (CalAim)

Status: Non-Exempt, Full-time

 

Job Summary:

This is a non-exempt position reporting to the assigned Program Manager for the area. The ECM Case Manager is an interdisciplinary, client-centered role which presumes responsibility for coordinating client needs through a variety of modalities. This position requires the application of evidence-based practices, knowledge of client barriers and social determinants of health, and an understanding of service coordination. This is a community-based position that interfaces with clients and care providers on a regular basis. This position also conducts community outreach, engages with other providers, conducts assessments, enrollments, and advocates on behalf of program clients. This position is required to travel throughout multiple counties within the region they are serving and be comfortable navigating roads and highways on a regular basis. 

Job Duties:

  1. Assures that the quality of ECM services is maintained throughout the client’s enrollment. Ensures all services are well coordinated and that client choice is implemented whenever possible.
  2. Creates and implements care management plans for enrolled clients.
  3. Identifies barriers to treatment unique to each individual client.
  4. Assists clients in navigating physical health, mental health, and other social health services within the region.
  5. Assists clients in attaining client-centered goals.
  6. Through care coordination practices, assists in coordinating appointments and accompanies clients to scheduled appointments when necessary.
  7. Conducts community outreach to increase community awareness of CalAim and ECM programs, and to reach additional eligible participants. 
  8. Monitors referrals and follows up as necessary to ensure clients are able to access services.
  9. Documents engagement frequency and ensures adequate case documentation in the Nation’s Finest Electronic Health Record.
  10. Incrementally documents all aspects of client care plans;
  11. Documents frequency of engagement and duration of encounters;
  12. Evaluates and documents client case details which factor into referral decisions;
  13. Maintains ROIs as appropriate;
  14. Documents client disengagement;
  15. Documents any client records obtained;
  16. Documents any other reporting metrics, client notes, and/or client or household data as required by the supervisor or contracting entity.


  1. Coordinates client care with other Nation’s Finest programs as appropriate.
  2. Review all incident reports. Consult with Site Director on any actions to be taken and implement changes to avoid similar incidents in the future.
  3. Inform the Program Manager in a timely manner of all relevant political, financial, and liability issues which arise and make recommendations for any changes in procedures. 
  4. Participate in internal and external committees and team meetings as assigned.
  5. Assists in the training of new ECM Case Managers.
  6. Review all reports of suspected abuse or neglect of vulnerable adults or minors and assure that appropriate actions are taken. Inform the Program Manager if reports involve alleged maltreatment by staff, students, or volunteers. 
  7. Maintain personal boundaries within Nation’s Finest policy and procedures.
  8. Other duties as assigned by the Program Manager or other supervisor.
  9. May require occasional over-time in accordance with Nation’s Finest policies.

Required Job Skills:

  1. Knowledge of HIPAA parameters.
  2. Ability to learn new software systems effectively.
  3. Knowledge of the laws and regulations governing behavioral health practice and procedures.
  4. Knowledge of best practices in case management for at-risk populations.
  5. Must have the highest ethical standards and the ability to articulate those standards to staff.
  6. Ability to work independently with minimal supervision.
  7. Intermediate level computer skills. Proficiency is required with the following applications: MS Word, Outlook, SharePoint, Teams, and Excel. Experience working with an Electronic Health Record system is highly preferred.
  8. Ability to work with a diverse population of clients in varying stages of addiction recovery and to be sensitive to their physical, mental and emotional health needs.
  9. Sensitivity to and awareness of cultural and linguistic diversity found in the service region.
  10. Knowledge of and ability to utilize peripheral services which may be necessary in addressing client needs.

Qualifications:

  • Bachelor’s degree in related field required – Experience may be substituted for education on a year-for-year basis.
  • At least two years’ experience serving an at-risk population is required. 
  • At least two years’ experience using Microsoft office applications is required (Word, Excel, Outlook, Teams, SharePoint.)
  • Ability to set and maintain clear boundaries with clients is required.
  • Valid driver’s license and insurance is required. No DUIs, reckless driving, etc. charges in last four years.
  • Knowledge of CalAim programs is highly preferred.
  • Bilingual in English and Spanish is preferred and pays an additional $1.00 per hour.

Offering

$33.00-$37.00 Per Hour ($58,344.00 - $65,416.00/year), DOE. Full-time position. Outstanding benefits package and ability to positively impact at-risk individuals in your community.

Position may require occasional night and/or weekend hours. Nation’s Finest offers a four-day workweek, and a casual, supportive work environment.
