Starting Hourly Pay: $22.37

Job Description

The ALTCS Case Manager ensures that appropriate and cost effective medical, medically related social and behavioral health services are identified, planned, obtained and monitored for members of ALTCS. NATIVE HEALTH is a contractor under the AHCCCS program to provide case management for enrolled tribal ALTCS members that reside on tribal reservations within the state of Arizona.  This position requires driving long distance to reservations located throughout the state for client case management and requires frequent overnight stays. This position reports to the ALTCS Program Manager

RESPONSIBILITIES AND KEY ACCOUNTABILITIES:

  1. Responsible for service planning and coordination identifying services that effectively meet the member’s needs in the most cost effective manner.
  2. Provides brokering of services to obtain and integrate all ALTCS services to be provided to the member, as well as other aspects of the member’s care, in accordance with the service plan.
  3. Facilitates and advocates to resolve issues which impede the member’s progress and access to needed services (both ALTCS and non-ALTCS covered services) ensuring services provided are beneficial for the member.
  4. Assesses the member’s placement, the services being received, and the cost effectiveness of care and takes necessary action to ensure that services and placement are appropriate to meet the member’s needs in the most integrated setting.
  5. Regularly collects data, monitors compliance, and produces program reports.
  6. Supports the organization as needed.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Undergraduate degree required in Nursing, Social Work, Gerontology, or similar field.
  2. Three years of case management experience in health or social services preferred.
  3. Understanding of the Native American community, culture, and its social and medical needs.
  4. Proficient with computer based applications such as MS Office, Adobe, Outlook, and other software programs.
  5. Possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years.  Must maintain a valid AZ driver license during employment. Must be insurable by NATIVE HEALTH liability auto policy. 
  6. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  7. Must submit to random testing during the course of employment.
  8. Must have an updated Immunization (IZ) Record, including COVID vaccination record. 
  9. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
  10. Must obtain a CPR Card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.

  3. Excellent written and verbal communication skills and ability to establish rapport.
  4. Positive attitude and the ability to advocate on behalf of the member’s needs in the clinical and social service environment.
  5. Contributes to team efforts by carrying out responsibilities in a positive and professional manner and by working to accomplish related goals.
  6. Good documentation skills, including detailed and accurate documentation.
  7. Medical terminology knowledge and understanding.
  8. Ability to work independently with strong sense of focus, task-oriented, non-judgmental, and clear sense of boundaries.
  9. A strong sense of and respect for confidentiality involving both clients and fellow employees.
  10. Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate.
